Ample research links mothers’ postpartum depression (PPD) to adverse interactions with their infants. However, most studies relied on general population samples, whereas a substantial number of women are at elevated depression risk. The purpose of this study was to describe mothers’ interactions with their 6- and 12-month-old infants among women at elevated risk, although with a range of symptom severity. We also identified higher-order factors that best characterized the interactions and tested longitudinal consistency of these factors from 6 to 12 (...) months of infant age. We leveraged data from eight projects across the United States (n = 647), using standardized depression measures and an adaptation of the NICHD Mother-Infant Interaction Scales. Overall, these depression-vulnerable mothers showed high levels of sensitivity and positive regard and low levels of intrusiveness, detachment, and negative regard with their infants. Factor analyses of maternal behaviors identified two overarching factors—“positive engagement” and “negative intrusiveness” that were comparable at 6 and 12 months of infant age. Mothers’ ability to regulate depressed mood was a key behavior that defined “positive engagement” in factor loadings. An exceptionally strong loading of intrusiveness on the second factor suggested its central importance for women at elevated depression risk. Mothers with severe depressive symptoms had significantly more “negative intrusiveness” and less “positive engagement” with their 6-month-old infants than women with moderate or fewer depressive symptoms, suggesting a potential tipping point at which symptoms may interfere with the quality of care. Results provide the foundation for further research into predictors and moderators of women’s interactions with their infant among women at elevated risk for PPD. They also indicate a need for evidence-based interventions that can support more severely depressed women in providing optimal care. (shrink)

A high level of citizen involvement in civic life is presumed crucial to the well-being of democracy, but the actual discourse of citizen involvement has rarely been analyzed. This article analyzes citizen participation in the school board meetings of one US community that was in the midst of conflict. After providing background on education governance practices and the community that was studied, citizen participation is examined. Citizen commentaries at school board meetings are shown to be a distinct speech genre and (...) the genre is described. Then, we characterize the discourse practices used to express negative sentiment. Feeling-limned description, avowal of feelings, rhetorical questions, reported speech, use of god and devil terms, and using meeting rules as weapons were the main negative sentiment strategies. In the conclusion we suggest when low levels of citizen involvement should be acceptable, and why `reasonable hostility' is a desirable form of citizen expression. (shrink)

Nursing Inquiry 2011; 18: 2–11 Demoralization and remoralization: a review of these constructs in the healthcare literatureDevelopment of the constructs of demoralization and remoralization began in the psychiatric literature in the 1970s when a psychiatrist in the USA observed a pattern of characteristics in people referred to him for depression, which he believed, was not depression. These characteristics included hopelessness, helplessness, isolation, low self-esteem and despair. Such characteristics are often termed existential distress. Distinguishing between depression (...) and the existential distress of demoralization is still central in the literature. This is important as successful responses to each condition differ. Research into these constructs has broadened and contributions now come from many different countries and multiple health disciplines, including nursing. This study presents a review and exploration of these constructs in the healthcare literature in an attempt to bring them to the attention of greater numbers of nurses. Facilitating remoralization requires time and sensitivity to people’s personal narratives. Difficulty in achieving such a response in the present efficiency climate of many health institutions can lead to moral distress. (shrink)

Major considerations of disability studies—such as provision of care, accommodation for disabled people, and issues surrounding institutionalization—have been consistently marginalized in American bioethical discourse. The right to die, however, stands out as a paradigmatic bioethical debate. Why do advocates for expanding the volition and self-direction of disabled people emerge from the periphery only to help those disabled people who choose death? And why do the majority of people assume an unrealistically low quality of life for those with disabilities? This paper (...) will argue that the dominance of the Western liberal tradition in American culture motivates both these phenomena: by emphasizing individual rights over duties and responsibilities, assuming the isolated and independent rights-bearer as the prototypical person, and evoking an unrealistically atomistic view of human interaction. As an alternative, I offer a framework rooted in feminist ethics that emphasizes context, gives moral weight to human relationships, abandons the problematic ideal of a lone rights-bearer, and emphasizes the mutual vulnerability of embodied individuals. Attachment orientations reflect individuals’ expectations for interpersonal relationships and influence emotion regulation strategies and coping. Previous research has documented that anxious and avoidant attachment orientations have deleterious effects on the trauma recovery process leaving these survivors vulnerable to posttraumatic stress disorder symptoms. However, avoidant attachment may be more complicated. Prior work has also found those high in avoidant attachment but also low in anxious attachment may not experience such vulnerabilities. Further, avoidant attachment individuals often report higher self-efficacy than their anxiously (...) attached counterparts. The present study examined trauma coping self-efficacy as a previously unexamined mechanism of action between adult attachment and PTSD symptoms. Structural equation modeling results showed that anxious attachment was associated with lower CSE-T and greater PTSD symptoms six weeks later. Further, a significant indirect effect of anxious attachment on PTSD symptoms through CSE-T was found. Contrary to hypotheses, avoidant attachment also exhibited an indirect effect on PTSD symptoms through CSE-T, such that avoidant attachment was associated with lower CSE-T, which in turn, was associated with greater PTSD symptoms. Also contrary to hypotheses, the interaction between anxious and avoidant attachment was not significantly associated with either CSE-T or PTSD symptoms. Results suggest that both anxious and avoidant attachment orientations contribute to poor self-regulation following trauma, as they undermine perceptions of trauma coping self-efficacy. (shrink)

Disturbances of consciousness, including fluctuations in attention and awareness, are a common and clinically important symptom in dementia with Lewy bodies. In the present study we investigate potential mechanisms of such disturbances of consciousness in a clinicopathological study evaluating specific components of the cholinergic system. [3H]Epibatidine binding to the high-affinity nicotinic receptor in the temporal cortex differentiated DLB cases with and without DOC, being 62–66% higher in those with DOC. The were no differences between DLB patients with or without DOC (...) in 125I-labeled α-bungaratoxin binding to the low-affinity nicotinic receptor, [3H]pirenzepine binding to the muscarinic M1 receptor, or in choline acetyltransferase activity. These findings provide support for the hypothesis that cholinergic activity is an important neural correlate if consciousness and suggest a mechanism of DOC in DLB involving alterations in the nicotinic receptor, composed of predominantly α4 and β2 subunits. As infancy is characterized by rapid physical growth and critical periods of development, disruptions due to illness or disease reveal vulnerability associated with this period. Spinal cord injury has devastating consequences at any age, but its onset neonatally, at birth, or within the first year of life multiplies its impact. The immediate physical and physiological consequences are obvious and immense, but the effects on the typical trajectory of development are profound. Activity-based restorative therapies capitalize on activity-dependent plasticity of the neuromuscular (...) system below the lesion and when provided to children with SCI aim to improve the child’s neuromuscular capacity, health and quality of life. This is a report of an infant with a cervical SCI at birth resulting in paralysis of leg and trunk muscles and paresis of arm and hands who was enrolled in an ABRT program at 3 years of age. After 59 sessions of ABRT, the child demonstrated significant improvements in trunk control and arm function, as well as social and emotional development. Despite the chronicity of injury and low expectations for improvement with therapeutic interventions, ABRT had a positive impact on the child’s physical capacity and provided benefits across multiple developmental domains. One obvious result of DNA exonerations has been the enactment of legislation regulating postconviction DNA testing. But the impact on our criminal justice system goes beyond formal statutory change. The DNA exonerations are changing attitudes towards the death penalty, are focusing attention on how forensic laboratories operate, and are leading to the stricter scrutiny of forensic science.

A collected volume of essays by renowned ethicist Margaret Farley, including articles previously published in scholarly periodicals as well as unpublished lectures and spiritual writings. There are vast ethical, legal, and social differences between natural death and euthanasia. In Death Talk Margaret Somerville argues that legalizing euthanasia would cause irreparable harm to society's value of respect for human life, which in secular societies is carried primarily by the institutions of law and medicine. Death has always been a central focus of the discussion that we engage in as individuals and as a society in searching for meaning in life. Moreover, we accommodate the inevitable reality (...) of death into the living of our lives by discussing it, that is, through "death talk." Until the last twenty years this discussion occurred largely as part of the practice of organized religion. Today, in industrialized western societies, the euthanasia debate provides a context for such discussion and is part of the search for a new societal-cultural paradigm. Seeking to balance the "death talk" articulated in the euthanasia debate with "life talk," Somerville identifies the very serious harms for individuals and society that would result from accepting euthanasia. Our physical ecosystem is not indestructible and we have obligations to hold it in trust for future generations. The same is true of our metaphysical ecosystem - the values, principles, attitudes, beliefs, and shared stories on which we have founded our society. In Bird on an Ethics Wire, Margaret Somerville explores the values needed to maintain a world that reasonable people would want to live in and pass on to their descendants. Somerville addresses the conflicts between people who espouse (...) "progressive" values and those who uphold "traditional" ones by casting her attention on the debates surrounding "birth" and "death" and shows how words are often used as weapons. She proposes that we should seek to experience amazement, wonder, and awe to enrich our lives and help us to find meaning. Such experiences, Somerville believes, can change how we see the world and live our lives, and affect the decisions we make, especially regarding values and ethics. They can help us to cope with physical or existential suffering, and ultimately put us in touch with the sacred - in either its secular or religious form - which protects what we must not destroy. Experiencing amazement, wonder, and awe, Somerville concludes, can also generate hope, without which our spirit dies. Both individuals and societies need hope, a sense of connection to the future, if the world is to make the best decisions about values in the battles that constitute the current culture wars. Intentionality is characteristic of many psychological phenomena. It is commonly held by philosophers that intentionality cannot be ascribed to purely physical systems. This view does not merely deny that psychological language can be reduced to physiological language. It also claims that the appropriateness of some psychological explanation excludes the possibility of any underlying physiological or causal account adequate to explain intentional behavior. This is a thesis which I do not accept. I shall argue that physical systems of a specific sort (...) will show the characteristic features of intentionality. Psychological subjects are, under an alternative description, purely physical systems of a certain sort. The intentional description and the physical description are logically distinct, and are not intertranslatable. Nevertheless, the features of intentionality may be explained by a purely causal account, in the sense that they may be shown to be totally dependent upon physical processes. (shrink)

Transnational feminism should have normative force and be anti‐imperialist. This article addresses the possibility of an anti‐imperialist transnational feminism in conversation with Serene Khader’s Decolonizing Universalism. Khader argues that the key to an anti‐imperialist feminism is separating universalism from the features that result in imperialism, such as ethnocentrism and justice monism. This article shares Khader’s commitment to anti‐imperialist feminism and further explores three relevant issues: human rights, the definition of feminism, and economic justice. It proposes a decolonizing view of rights (...) that advocates a critical examination of their uses, applications, and origins. It argues that transnational feminism should engage with issues of economic justice. And it proposes that feminism as an intersectional praxis should include normative dimensions in addition to antisexism, such as antiracism and economic justice. (shrink)

ABSTRACT This is a personal reflection on what I have learnt as an academic, researching, teaching and participating in the public square in Bioethics for over four decades. I describe a helix metaphor for understanding the evolution of values and the current “culture wars” between “progressive” and “conservative” values adherents, the uncertainty people’s “mixed values packages” engender, and disagreement in prioritizing individual rights and the “common good”. I propose, as a way forward, that individual and collective experiences of “amazement, wonder (...) and awe” have the power to enrich our lives, help us to find meaning and sometimes to bridge the secular/religious divide and experience a shared moral universe. They can change our worldview, our decisions regarding values and ethics, and whether we live our lives mainly as just an individual – a “me” – or also as a member of a larger community – a “We”. I summarize in an equation – “The Wonder Equation” – what is necessary to reduce or resolve some current hostile values conflicts in order to facilitate such a transition. It will require revisiting and reaffirming the traditional values we still need as both individuals and societies and accommodating them with certain contemporary “progressive„ values. (shrink)
